remote ischemic preconditioning in lower limb surgery; the hemodynamic and respiratory effects

چکیده
aim and background: remote ischemic preconditioning introduces brief episodes of ischemia and reperfusion which reduces long term ischemia in orthopaedic surgery. the aim of this study was to evaluate hemodynamic and respiratory effects of remote ischemic preconditioning in lower extremity orthopaedic surgeries. methods: in this clinical trial 40 patients scheduled for lower extremity surgery with pneumatic tourniquet were randomly allocated to remote ischemic preconditioning (rip group, n=20) and control group (n=19). patients in rip group received three “5 minutes” cycles of ischemia, alternating with 5 minutes of reperfusion before extended use of tourniquet. hemodynamic variables prior to inflation of tourniquet, every 30 minutes during the surgery and 10 minutes after tourniquet deflation and also arterial blood gas sample prior to and after surgery were recorded and compared between groups. results: during operation blood pressure dropped in the rip group and variations in heart rate, respiratory rate and pulse oximeter measurements after surgical tourniquet release were not significantly different between two groups. changes in blood gas parameters were significantly less pronounced in the rip group. conclusion: remote ischemic preconditioning may not attenuate most of the adverse effects of surgical tourniquet deflation including variations in heart rate, respiratory rate and arterial oxygen saturation as well as blood pressure drops. however, rip may reduce increases in systolic blood pressure and acidosis following tourniquet application. key words: remote ischemic preconditioning; orthopedic, surgery, blood pressure, tourniquet, oxygenation

Effects of remote limb ischemic preconditioning on pulmonary function tests and maximal oxygen uptake
Introduction: Ischemic preconditioning (IPC) protects skeletal muscles from ischemia-reperfusion injury and improves physical exercise performance. The purpose of the present study was to determine whether application of remote ischemic preconditioning (RIPC) of upper limbs would affect the pulmonary function tests and the maximal oxygen consumption (VO2max). Methods: Twenty healthy trained...
متن کاملRemote Ischemic Preconditioning and Diabetic Macular Edema
Objective: Remote Ischemic Preconditioning (RIPC) as the transient ischemia and reperfusion of the arm is a promising method for protecting different tissue from future ischemia. These effects might be mediated through vascular and endothelial growth factor (VEGF) pathway. We investigated the influence of RIPC on diabetic macular edema (DME) as a chronic ischemic condition in patients who were ...
متن کاملRemote Ischemic Preconditioning and Cardiac Surgery.
To the Editor: Remote ischemic preconditioning (RIPC) is an inexpensive technique that is used to reduce perioperative organ damage. High-quality randomized trials have shown cardiac1 and renal2 protection with a possible reduction in mortality when RIPC was used in addition to halogenated agents.3 However, two recent multicenter, randomized trials, the Remote Ischemic Preconditioning for Heart...
متن کاملReduction of hemorrhagic shock–induced acute kidney injury by lower limb ischemic preconditioning in rats
Introduction: During hemorrhagic shock (HS), the kidneys are one of the primary target organs involved. Oxidative stress is shown to be enhanced in different models of acute kidney injury (AKI). Remote ischemic preconditioning (RPC) by brief limb ischemia is considered to be a safe method to protect different organs from further damage. In this study, we investigated the effects of brief hind l...

Aim and Background: Arterial tourniquets are used widely for extremity orthopedic surgeries with regional or general anesthesia to reduce blood loss, but the harmful effect of ischemia and reperfusion is not clear yet. Materials and Methods: In this clinical trial 40 patients scheduled for lower extremity surgery with pneumatic tourniquet were randomized into 2 groups: a remote ischemi...
